On or about November 7, 2018, during the removal of interior wall and ceiling material at a job site located at 925 Keynote Cir, Brooklyn Heights, Ohio 44131; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces of approximately 40 feet.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.     On or about November 7, 2018, during the removal of interior wall and ceiling material at a job site located at 925 Keynote Cir, Brooklyn Heights, Ohio 44131; the employer did not ensure employees working on the 5th floor of a commercial building were protected from fall hazards (40 feet above the ground) while passing discarded material through a wall opening by utilizing a PFAS or protected by any other form of fall protection system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     On or about November 7, 2018, during the removal of interior wall and ceiling material at a job site located at 925 Keynote Cir, Brooklyn Heights, Ohio 44131; the employer did not ensure the companys employees working on the 5th floor of a commercial building were protected from a fall of approximately 40 feet by utilizing a fall protection system to include providing fall protection training or fall hazard recognition.
