29 CFR 1926.856(b): During demolition operation(s), floor openings did not have curbs or stop-logs to prevent the mechanical equipment from running over the edge: On or about November 13, 2018 during construction activities including, but not limited to, demolition activities at a job site located at 3700 Limestone Dr, Barberton, Ohio 44203; the employer did not ensure employees operating 2 Bobcat E35i Mini Excavators (with hammer attachment) were protected from driving into/off the edge of the roof and or floor opening by ensuring there were curbs and or stop-logs in place.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. On or about November 13, 2018 employees were observed working on the South side of the roof (standing near the edge, operating a robotic hammer) of a Limestone storage silo without the use of PFAS or protected by any other form of fall protection system. The employees were part of a work crew in the demolition process of hammering/cutting/breaking apart the cement and rebar support portion of the roof. The roof height was approximately 155 feet above the ground. The three employees were exposed to fall hazards of approximately 130 to 155 feet above the ground on all exterior sides and edges of the silos. b. On or about November 13, 2018 employees were engaged in the demolition process of hammering/cutting/breaking apart the cement and rebar support portion of the roof of a Limestone storage silo without the use of PFAS or protected by any other form of fall protection system. Employees were exposed to 20 uncovered 18 inch by 18 inch vent shafts. The three employees were exposed to fall hazards of approximately 130 to 155 feet above the ground. c. On or about November 13, 2018 employees were engaged in the demolition process of hammering/cutting/breaking apart the cement and rebar support portion of the roof of a Limestone storage silo without the use of PFAS or protected by any other form of fall protection system. Employees were exposed to unprotected interior edges of the demolished roof. The tall 3 silos (155 feet above the ground), had approximately 1,580 sf of 2,700 sf total surface demolished creating the fall hazards. The Low 9 silos (130 feet above the ground), had approximately 4,894 sf of 8,100 sf total surface demolished creating the fall hazards.
