1926.20(b)(2) The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying exis(a)(LOCATION)(IDENTIFY THE HAZARDOUS CONDITION AND THE DEFICIENT ACTION) 29 CFR 1926.850(i): All floor openings, not used as material drops, shall be covered over with material substantial enough to support the weight of any load which may be imposed. Such material shall be properly secured to prevent its accidental movement. a) Job site, 400 5th Avenue, Pittsburgh, PA, 15219: On or about January 26, 2021, the employer did not ensure all floor openings were adequately covered in order to prevent employees from falling while in the process of conducting demolition activities.

29 CFR 1926.502(i)(4):All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: a) Job site, 400 5th Avenue, Pittsburgh, PA: On or about January 26, 2021, during demolition activities, the employer did not ensure that the cover over a hole in the concrete floor was color coded or marked to identify that there was an open hole below the cover, thus exposing employees to a fall hazard of approximately 15 feet.
