29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  San Francisco Presidio, Building No. 41: An employee was observed with their foot on the mid-rail of the JLG aerial lift basket and the other foot on the top rail of the basket. The employee was not tied off to the basket and was exposed to a fall of 13 feet to the ground below.

29 CFR 1926.453(b)(2)(v): A body belt or harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:   San Francisco Presidio, Building No. 41: Two employees of were observed working from the basket of the JLG aerial lift, about 13 ft. above the ground, without attaching their body harnesses to the basket. The employees were exposed to the fall hazards.
