29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: On or about November 27, 2018, at the construction site where employees were exposed to a fall hazard while working on an elevated scaffold that was not attached to the forklift.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about November 27, 2018, at the construction site where employees were exposed to a fall hazard of more than 15 feet while working on an elevated scaffold without a fall protection system in place.
