(b)    Blast Booth:  On or about November 30, 2018 employees that go on top of the blast booth were using a fall harness that had only one lanyard to attach to a 32-inch high guardrail with posts, as instructed by management, which requires the employee to detach his lanyard to go down the length of the guardrail.
