29 CFR  1910.37(b)(2):Each exit was not clearly visible and marked by a sign reading "Exit":   On or about March 10, 2021, and times prior thereto, in the office building the employer did not ensure exit doors were marked with an "EXIT" sign exposing employees to fire and smoke hazards.  a)  The front entrance  designated exit door without an "EXIT" sign. b) The north designated exit door without an "EXIT" sign.

29 CFR  1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:    On or about March 10, 2021, and times prior thereto, in the office building the employer did not place "EXIT" directional travel signs throughout the corridor so employees could exit the building quickly in case of an emergency.  Employees were exposed fire and smoke hazards.

On or about March 10, 2021, and times prior thereto, in the cement linen building, the employer did not ensure openings in electrical panel boxes were effectively closed with rubber or metal plugs, exposing employees to electrical and fire hazards:  a)  In the cement pump control room, electrical box missing knockouts. b)  At the DC Spirit Plus electrical box missing knockouts on the right side.

29 CFR  1910.184(c)(6): Slings were not securely attached to their loads:    On or about March 10, 2021, and times prior thereto, in the fabrication building, the employer did not ensure safety latches on lifting hooks were serviceable to secure loads while lifting and lowering, exposing employees to struck-by hazards.  a) At station number 1, 2 ton hoist with unserviceable safety clip. b) At station number 2, 1 ton hoist with broken clip.
