29 CFR 1915.89(c)(1): The employer did not ensure that, before any authorized employee performed servicing when energization, startup, or the release of hazardous energy could occur, that all energy sources were identified and isolated, and the machinery, equipment, or system was rendered inoperative:    USS Miguel Keith, STBD Casing 1-34-1: On November 28, 2018, employees were performing a continuity check on a panel containing energized components. Prior to starting the work, all energy sources were not identified and isolated. An employee contacted two different phases of the line side concurrently with a set of alligator clips, causing an arc flash. The employee was exposed to electrical shock and/or burn hazards from 480 volts.

29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE.     USS Miguel Keith, STBD Casing 1-34-1: On November 28, 2018, an employee was performing a continuity check on a panel with energized components while wearing a hard hat, safety glasses, hearing protection, and safety shoes. Electrical-related personal protective equipment for the face and neck was not utilized. The employee was exposed to electrical shock and/or burn hazards from 480 volts.

29 CFR 1915.152(b): The employer did not assess its work activity to determine whether there were hazards present, which necessitate the employee's use of personal protective equipment:    USS Miguel Keith, STBD Casing 1-34-1: On November 28, 2018, an employee was performing a continuity check on a panel with energized components within inches of the work. The employer's hazard assessment for working on high voltage electrical equipment did not include protection for adjacent exposed energized parts that could be accidentally contacted.  The employee was exposed to electrical shock and/or burn hazards from 480 volts when he inadvertently made contact with energized parts of the panel.
