On or about December 6, 2018, employees were exposed to a fall of approximately 6 feet while installing vinyl siding materials on a new residential home located on SL 123 Timberlane Trail, Hudson, Ohio 44236.  Employees were using an A-Frame ladder on the roof in the back of the home and the employer did not ensure that they were using it for the purpose that it was designed.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about December 6, 2018, employees were exposed to a struck by hazard while installing vinyl siding materials on a new residential home located on SL 123 Timberlane Trail, Hudson, Ohio 44236.  Employees were working on upper levels using materials and tools that could have fallen onto employees working below and the employer did not ensure that the lower employees were wearing protective helmets.     Trademark Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.100(a), which was contained in OSHA inspection number 1131211, citation number 1, item number 1 and was affirmed as a final order on 4/6/2016, with respect to a workplace located at 11768 Valley View, Sagamore Hills, Ohio 44067.

On or about December 6, 2018, employees were exposed to a fall of approximately 12 feet while installing vinyl siding materials on a new residential home located on SL 123 Timberlane Trail, Hudson, Ohio 44236.  The employees were working from a ladder jack scaffold and the employer did not ensure that they were using fall protection.     Trademark Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.451(g)(1)(vii), which was contained in OSHA inspection number 1131211, citation number 1, item number 2 and was affirmed as a final order on 4/6/2016, with respect to a workplace located at 11768 Valley View, Sagamore Hills, Ohio 44067.

Two employees were working on the roof in the rear of the home and the employer did not ensure that they were using any form of fall protection.    Trademark Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 1131211, citation number 1, item number 4 and was affirmed as a final order on 4/6/2016, with respect to a workplace located at 11768 Valley View, Sagamore Hills, Ohio 44067.
