29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    On or about September 12, 2019, the employer failed to ensure that each employee who performed work while on an approximately 14 feet 11.5 inch high ladder jack scaffold with no fall protection was trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.

The employer failed to provide a training program for each employee who might be exposed to fall hazards.

Trademark Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 1364175, citation number 2 item number 2 and was affirmed as a final order on January 22, 2019, with respect to a workplace located at SL 123 Timberlane Trail, Hudson, Ohio 44236.    Trademark Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(vii), which was contained in OSHA inspection number 1131211, citation number 1, item number 2 and was affirmed as a final order on April 6, 2016 with respect to a workplace located at 11768 Valley View, Sagamore Hills, Ohio 44067.
