29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift: On or about November 19, 2018, at the Henderson Mill Mall Parking Lot 2078-2186 Henderson Mill Rd, Atlanta, Georgia: An employee performing light bulb replacement was exposed to a fall hazard of approximately 20-30-feet while working from the basket of a Genie aerial lift without wearing a harness and tying off to the basket of the lift.
