29 CFR 1910.303(b)(1)(iii) as required by 29 CFR 1960.8(b):  Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the consideration of wire-bending and connection space:  (a)  Second floor break room; Electrical cord for the GE microwave was positioned behind a cabinet in such a way that the cord was severely bent back on the plug, thereby subjecting the wires to damage.

29 CFR 1910.305(g)(1)(iv)(A) as required by 29 CFR 1960.8(b):  Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  (a) Second floor; A laptop and docking station were plugged into a relocatable power tap that was being powered by an extension cord.  (b) First floor kitchen/break area; A Kenmore microwave oven and a Toastmaster four slice toaster were plugged into a relocatable power tap instead of permanent receptacles.  (c) Mississippi Park Connection area; A Lasko moveable air heater and a Holmes space heater were plugged into relocatable power taps instead of permanent receptacles.
