29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a) Facility, 2211 Memorial Highway, Dallas, PA - Employees operated Crown Sit-Down Rider Truck Lift Model FC4020-60 (Serial No. 9A142157) with a front-end attached remanufactured Cascade Paper Roll Clamp Model 45F-RCP-03R (Serial No. 894195-T1) without the manufacturer's prior written approval exposing employees to struck-by hazards created by the modifications to the powered industrial truck's lifting capacity and center of gravity, on or about December 18, 2018.  b) Facility, 2211 Memorial Highway, Dallas, PA - Employees operated Crown Sit-Down Rider Truck Lift Model FC4520-60 (Serial No. 9A200474) with a front-end attached Cascade Paper Roll Clamp Model 22H-RCP-B003 (Serial No. 2377125-1RO) without the manufacturer's prior written approval exposing employees to struck-by hazards created by the modifications to the powered industrial truck's lifting capacity and center of gravity, on or about December 18, 2018.  In accordance with 29 CFR 1903.19, abatement certification is required for this violation.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  a) Facility, 2211 Memorial Highway, Dallas, PA - Employees operated Crown Sit-Down Rider Truck Lift Model FC4020-60 (Serial No. 9A142157) with a front-end attached remanufactured Cascade Paper Roll Clamp Model 45F-RCP-03R (Serial No. 894195-T1) to transport paper rolls on a daily basis. The forklift lacked allowable attachment markings which exposed employees to fall hazards and struck-by hazards due to the unknown modifications to the forklift's lifting capacity and center of gravity created by the use of the clamp, on or about December 18, 2018.  b) Facility, 2211 Memorial Highway, Dallas, PA - Employees operated Crown Sit-Down Rider Truck Lift Model FC4520-60 (Serial No. 9A200474) with a front-end attached Cascade Paper Roll Clamp Model 22H-RCP-B003 (Serial No. 2377125-1RO) to transport paper rolls on a daily basis.
