29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a) Facility, 2211 Memorial Highway, Dallas, PA - Employees operated Crown Sit-Down Rider Truck Lift Model FC4020-60 (Serial No. 9A142157) with a front-end attached remanufactured Cascade Paper Roll Clamp Model 45F-RCP-03R (Serial No. 894195-T1) without the manufacturer's prior written approval exposing employees to struck-by hazards created by the modifications to the powered industrial truck's lifting capacity and center of gravity, on or about December 18, 2018.  b) Facility, 2211 Memorial Highway, Dallas, PA - Employees operated Crown Sit-Down Rider Truck Lift Model FC4520-60 (Serial No. 9A200474) with a front-end attached Cascade Paper Roll Clamp Model 22H-RCP-B003 (Serial No. 2377125-1RO) without the manufacturer's prior written approval exposing employees to struck-by hazards created by the modifications to the powered industrial truck's lifting capacity and center of gravity, on or about December 18, 2018.  In accordance with 29 CFR 1903.19, abatement certification is required for this violation.
