The employer fails to assure effective protections against hazards associated with flying objects as required by the standard, as demonstrated on December 18, 2018, when employee #1 was operating a pneumatic nail gun attaching shingles to the roof of the garage without wearing a means of PPE, such as safety glasses.

The employer fails to assure effective protections against falls as required by the standard, as demonstrated on December 18, 2018, when male employee #1 (Forrest Daigle) wearing a black knit cap, red shirt, black jeans, and brown boots was working on the edge of the garage roof nailing shingles. The employee was not using any means of fall protection while working on the roof.
