29 CFR 1926.451(f)(6): Scaffolds are erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, came closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):    9 Pine Street  - The ladder jack scaffolding platform was less than three feet away from the exposed power lines and the lines were not deenergized or protected.     The employer fails to ensure effective protections against electrical hazards as required by the standard, as demonstrated on May 23, 2019, when employees were utilizing a scaffolding erected within approximately twelve inches from the residence's electrical service.

29 CFR 1926.501(b)(13): The employer does not ensure that each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are protected by guardrail systems, safety net systems, or personal fall arrest systems, or  provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):                                         9 Pine  Street - Employees performing roofing work on a roof with an 11:12 pitch were not protected from fall hazards by the use of a fall protection system. This condition exposed employees to a 21 foot, 4 3/4 inch fall hazard.  The employer fails to ensure effective protections against falls as required by the standard, as demonstrated on May 23, 2019, when three employees were working on a residential building installing shingles.  The employees were not using any means of fall protection while working on the roof.       Purvis Home Improvement, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1296851, citation number 2, item number 1 and was affirmed as a final order on 09/24/2018, with respect to a workplace located at 79 Mussey Road, Scarborough, Maine 04074.           Purvis Home Improvement, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1046978, citation number 2, item number 1 and was affirmed as a final order on 04/13/2015, with respect to a workplace located at 2 Avenue 3, Pine Point, Maine 04074.        Purvis Home Improvement, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 732963, citation number 1, item number 1 and was affirmed as a final order on 01/14/2013, with respect to a workplace located at Fieldings Oil Company, Scarborough, Maine 04074.           Purvis Home Improvement, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 392602, citation number 1, item number 1 and was affirmed as a final order on 08/16/2012, with respect to a workplace located at 607 Roosevelt Trail, Westbrook, Maine 04092.           Purvis Home Improvement, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 310101662, citation number 1, item number 3 and was affirmed as a final order on 10/13/2006, with respect to a workplace located at 556 Main Street, South Portland, Maine 04106.

29 CFR 1926.1053(b)(1): The employer does not ensure that where portable ladders are used for access to an upper landing surface and the ladder's length allows, the ladder side rails extend at least 3 feet (.9 m) above the upper landing surface being accessed:    9 Pine Street - The ladder being used to access the roof did not extend at least three feet above the roof's surface.  This condition exposed employees to a fall height of 10 feet, 2 inches.   The employer fails to ensure effective protections against fall hazards as required by the standard, as demonstrated on May 23, 2019, when employees were exposed to fall hazards where a ladder did not extend three feet above the roof's surface to allow for safe transition onto the roof.   Purvis Home Improvement, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1046978, citation number 1, item number 1 and was affirmed as a final order on 04/13/2015, with respect to a workplace located at Avenue 3, Pine Point Maine 04074.
