29 CFR 1910.23(c)(3): The employer did not ensure that ladders were not loaded beyond the maximum intended load:    a.)     On or about November 22, 2018, the employer did not ensure employees were protected while using a fiberglass A frame ladder. Employees are required to use a ladder to change the front sign and exceeded the capacity of the ladder, exposing them to fall hazards of up to 8 ft.

29 CFR 1910.23(c)(4): Unsecured ladder(s) were used on surfaces that were not stable and level:     a.)     The employer did not ensure employees were protected while using a fiberglass A frame ladder to change letters on the front sign. On November 22, 2018, an employee was attempting to change the sign using a ladder set in a stone area when the ladder became unstable and fell, resulting in arm injuries after falling approximately 8 to the stones below.
