29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.    Lake Erie Tree Service, LLC - Oregon, Ohio: On or about January 9, 2019, the employer failed to report an employee injury that resulted in an amputation.  On November 12, 2018, an employee sustained a partial finger amputation injury while operating a log splitter; the employer did not report the amputation within twenty-four hours.
