29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about January 7th, 2019, LS Construction, LLC, as the correcting and exposing employer did not ensure the ladders located in the front and back of the house were properly extended to reach 3 feet above the upper landing surface of the roof lower roofs thus exposing employees to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about January 7th, 2019, LS Construction, LLC, as the controlling and exposing employer did not ensure employees of sub-contractor, Heaven and Earth Construction, LLC were protected from fall hazards while engaged in re-roofing construction. Employees were exposed to falls from a height greater than approximately 25 feet and supervisor of LS Construction, Eduardo Gonzalez, did not stop work until fall protection was provided.
