29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii).   a)  On or about January 14, 2019, Metallurgical Services Incorporated., did not develop a written hazard communication program for employees exposed to but not limited to, Starblast (irritation to eyes, skin, throat and upper respiratory system). An employee was exposed to respirable dust when conducting sandblasting operations using the abrasive material Starblast.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) On or about January 14, 2019, Metallurgical Services Incorporated., did not provide employees training on hazardous chemicals in their work area. Employees conducting sandblasting operations were exposed to Starblast blasting abrasive (respirable dust). Exposure to respirable total dust can cause irritation to the eyes, skin, throat, and upper respiratory system.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form.  a) On or about January 14, 2019, Metallurgical Services Incorporated., did not evaluate the respiratory hazards such as but not limited to, respirable dust in the work place. An employee conducting sandblasting operations was exposed to a TWA of respirable dust of 0.224 mg/m3. Exposure to respirable total dust can cause irritation to the eyes, skin, throat, and upper respiratory system.
