29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (a) The employer did not develop specific requirements for testing machinery, equipment and manufacturing processes, to determine and verify the effectiveness of the lockout devices and to prevent inadvertent start-up. This exposed employees working on and around the equipment to caught-in and crushing injuries.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees. (a) On or about January 15, 2019, employees working on the Unitrol Solution Welder (Machine BW1, Model #9280D-602 & Serial #S0400508) were exposed to caught-in and crushing hazards while servicing and maintaining equipment. Employees were not provided adequate training to ensure that the purpose and function of the energy control program was understood and how to implement machine specific procedures.

The point of operation was not properly guarded. (b) On or about January 15, 2019, employees were exposed to crushing/pinch point hazards and amputation injury while working on and around the TAM70 machine. The point of operation was not properly guarded and the interlock was by-passed. (c) On or about January 15, 2019, employees were exposed to crushing/pinch point hazards and amputation injury while working on and around the BAM40 machine. The point of operation and ingoing nip points were not properly guarded. (d) On or about January 15, 2019, employees were exposed to crushing/pinch point hazards and amputation injury while working on and around the BAM20 machine. The point of operation and ingoing nip points were not properly guarded.
