29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard were being followed.    On or about 1/15/2018, Loy-Lange Box Company did not conduct a periodic inspection of the energy control procedures at least annually to ensure that the procedures and the requirements of this standard were being followed.  The following hazardous conditions existed at the time of the inspection:    a)  Periodic inspections were not conducted on their authorized employees to identify and correct deficiencies or inadequacies.  b)  Each energy control procedure had not been separately inspected since 10/24/2017 to ensure that the energy control program was being properly utilized.      Loy-Lange Box Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 1221846, citation number 2, item number 1 and was affirmed as a final order on 10/19/2017, with respect to a workplace located at 222 Russell Boulevard, Saint Louis, MO.    Loy-Lange Box Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 958460, citation number 1, item number 1a and was affirmed as a final order on 04/11/2004, with respect to a workplace located at 222 Russell Boulevard, Saint Louis, MO.
