29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.  In the production area, on or about 12/29/18:  Employee fabricating couplings on a "jump shear" when the unguarded blade contacted their fingertip resulting in an amputation.  Abatement documentation must be submitted for this item.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a.  At the facility, on or about 1/17/19:  On 12/26/18, an employee suffered an amputation of a fingertip from contact with a shear.  The employer did not report the amputation until 1/11/19, 16 days later.
