29 CFR 1926.601(b)(4)(ii): The vehicle was backed up without the observer giving the signal that it was safe to do so: On or about January 17, 2019, an employee was exposed employee to struck-by and caught in between hazards while giving the hand signals to a concrete mixing truck driver. As the driver backed up his concrete mixer to the other truck he lost sight of the signaler but continued to back-up and pinned the signaler between both trucks.
