29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. On or about December 12, 2018, at jobsite located at 11158 Southern Blvd, Royal Palm Beach, FL 33411, employees were exposed to a fall hazards of approximately 18 feet while walking/working on a concrete wall without means of fall protection.    b. On or about January  25, 2019, at jobsite located at 11158 Southern Blvd, Royal Palm Beach, FL 33411, employees were exposed to fall hazards of approximately 18 feet while walking/working on a concrete wall without means of fall protection.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:    On or about December 12, 2018, at the jobsite located at 11158 Southern Blvd, Royal Palm Beach, FL 33411, the employer did not train employees in the safe usage of ladders, exposing employees to a fall hazard of 5 feet while using a Werner aluminum 24 ft. extension ladder leaning against the wall, and carrying in both hands a 4x8 plywood sheet to access the top of a wall.
