29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: Employees were exposed to fall and amputation hazards. Employees were permitted to walk and work near and around floor holes that did not have covers or railings. Employees were exposed to injuries from fall hazards in the following locations: A) The floor hole near the SE corner in the building near the booth. The opening contains a chain driven auger. Approximate dimensions are 56 1/2 inch width, 48 inch length, and a depth of 19 inches. B) The floor hole near the NE corner in the building near the booth. Approximate dimensions are 8 inch width, 16 inch length, and a depth of 15 inches.

29 CFR 1910.28(b)(6)(i): The employer did not ensure each employee less than 4 feet (1.2 m) above dangerous equipment was protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system when the equipment was not covered or guarded to eliminate the hazard: Employees were exposed to fall and amputation hazards. Employees were permitted to walk and work near and around the floor hole that contains a chain driven auger in the spray booth building. The floor hole did not have covers or railings, employees were exposed to injuries from the fall hazards.

29 CFR 1910.219(c)(4)(i): Shaft ends projecting more than one-half the diameter of the shaft were not guarded by nonrotating caps or safety sleeves. Employees were exposed to entanglement hazards. Employees were exposed to the rotating shaft end on the drive mechanism for the sand/shot reclamation auger of the booth. the shaft diameter was 1 and 1/8 inches and it projected out 1 and 15/16 inches.
