29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official were not marked on plates of approved design which were supplied and securely affixed by the owner of the building, or the duly authorized agent, in a conspicuous place in each space to which they related: On or about January 30, 2019 at the Laclede Chain Manufacturing facility- The approved load for the mezzanine in the center of the main production shop used for storage did not have the approved load rating conspicuously posted.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 30, 2019 at the Laclede Chain Manufacturing facility- Employee was exposed to a rotating part from the Die Quip Lathe (M2200), in that the rotating part was not covered or guarded adequately enough to prevent employee from being caught-in machine.
