29 CFR 1910.305(g): Flexible cords and cables -- 29 CFR 1910.305(g): Identification, splices, and terminations. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. On or about February 8, 2019, the electrical power supply providing power from the ceiling "drop" to the control panel box for the Viking QL306 was not provided strain relief protection where flexible cable entered the box. The cable was pulled out of the strain relief protection. Employees had exposure to this electrical shock hazard during maintenance activities associated with the Viking QL306 machine.

29 CFR 1910.95(d): "Monitoring." 29 CFR 1910.95(d)(3): Monitoring shall be repeated whenever a change in production, process, equipment or controls increases noise exposures to the extent that: On or about February 8, 2019, the employer failed to perform noise monitoring of pallet manufacturing operations when changes in production and equipment were made to determine if required hearing protection utilized was adequate. The pallet making production operations were changed, to include new equipment, following a workplace fire that resulted in the complete reconstruction of company manufacturing facilities.

29 CFR 1910.147(c): General. 29 CFR 1910.147(c)(4): Energy control procedure. 29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; On or about February 8, 2019, the employer's specific procedure for the Viking QL306 did not address stored mechanical and gravitational energy associated with machine mechanisms and the material those mechanisms process.
