29 CFR 1926.502(b)(13): When guardrail systems were used around holes which were used as access (such as ladderways), they were not provided with a gate, or were not so offset that a person could not walk directly into the hole:   (a) One Lakeside Apartment Project: On or about January 31, 2019 the roof access area next to the ladder was not guarded or offset to prevent a potential fall.  Abatement certification is required for this item.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement:   (a)   One Lakeside Apartment Project: On or about January 31, 2019 employees and subcontractors were exposed to hazards associated with impalement on uncapped, waist-high rebar that had been erected in the center of the working deck.
