29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:        a) Bow thruster room: on or about 04 February 2019; employees accessed a fixed ladder from a platform measuring approximately 40 inches wide, 6 feet from the lower deck, with an unprotected opening approximately 20 inches wide.  This condition exposed employees to fall hazards.         b) Weather deck, mid-crane: on or about 04 February 2019; crane operators accessed a platform measuring approximately 8 feet above the main weather deck without fall protection exposing them to fall hazards.
