29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Engineering workshop: On or about March 4, 2025, and at times prior thereto, when employee(s) used the Optimum lathe, SN J620244, the spindle was not guarded. This potentially exposed employee(s) to entanglement hazards.

29 CFR  1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Engineering workshop: On or about March 4, 2025, and at times prior thereto, employees utilizing the right side grinding wheel for the Dayton 10" Bench Grinder, model 2LKT2 were exposed to projectile and/or amputations hazard(s). The employer did not ensure that adjustable tongue for the right side grinding wheel was installed and adjusted  with a maximum opening of one fourth inch.
