29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    a) Engine room, refrigeration flat, area of removed compressor pump: on or about 07 February 2019; the refrigeration flat measured approximately 67 inches above the engine flat with an unprotected opening of approximately 24 inches wide exposing employees to fall hazards.     b) Engine room, refrigeration flat fixed ladder to engine flat: on or about 07 February 2019; the refrigeration flat measured approximately 67 inches above the engine flat had an unprotected edge of approximately 24 inches wide between the fixed ladder hand railing and adjacent equipment. This condition exposed employees to fall hazards.
