On or about February 12, 2019 at the El Paso Western Refining Facility, the employer failed to ensure that written procedures for testing interlocks were established and implemented such as but not limited to:      a) Interlock INT 301-5 Spent Cat Slide Valve Low Differential Shutdown (P&ID 300-011-10025-817 Rev.12).    b) Interlock INT 301-2 Low Air Flow to Regenerator Heater H-3000 (P&ID 300-011-10025-811 Rev.11).

On or about February 12,2019 at the El Paso Western Refining Facility, the employer did not perform inspections and testing on the following process equipment:    a) Intercat Main Catalyst Feed Tank (F-353).      b) Pressure Safety Valve PVS- W16233 located on the top of the Intercat Main Catalyst Feed Tank (F-353).      c) Interlocks:        1. Interlock INT 301-5 Spent Cat Slide Valve Low Differential Shutdown (P&ID 300-011-10025-817 Rev.12).        2. Interlock INT 301-2 Low Air Flow to Regenerator Heater H-3000  (P&ID 300-011-10025-811 Rev.11).    d) Controls and Indicators:           1. Level Control LC-258 (P&ID 300-011-10025-817 Rev.12).        2. Differential Pressure Indication PDI-225 (P&ID 300-011-10025-817 Rev.12).        3. Temperature Control TC-178 (P&ID 300-011-10025-811 Rev.11).        4. Fire Eyes BA-300A (P&ID 300-011-10025-811 Rev.11).

On or about February 12, 2019 at El Paso Western Refining Facility, the employer did not ensure that piping and instrument diagrams (P&IDs) were accurate.  The P&IDs were not updated and accurate for the following:      a) P&ID 300-011-10025-810 Rev. 15, did not include the Fresh Cat Loader, Diesel Compressor, Drier Skids, Boiler House, FCC Loop, and Intercat Additive.      b) P&ID 130-011-10025-830 Rev. 13, did not include the Chemical Injection Point.      c) P&ID 300-011-10025-V01 Rev. 02, did not include line connecting tanks F-354 and F-355 to the process.

On or about February 12, 2019 at the El Paso Western Refining Facility, the employer did not inform employees by posting danger signs or by any other equally effective means at locations containing permit spaces for the following:    a) LCO Stripper (F-1301) skirt in the Fluidized Catalyst Cracking Unit (FCCU).    b) Off Gas Amine Column (F-1311) skirt in the FCCU.     c) Second stage reactor (F-8906) skirt in the Catalyst Gasoline Hydrotreater (CGHT).    d) Vessel F-8140 skirt in the Alkylation unit.
