29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  Southeast corner of Warehouse #79: A cut-off switch to a 120 volt electrical panel box used to control lighting and receptacle outlets was blocked by storage consisting of discarded portable fire extinguishers, a barrel floor fan, cardboard boxes full of miscellanies items, wooden skids, and other debris.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables were run through doorways, windows, floors, or similar openings.  a) Warehouse Building #79 - Supervisor's office West side window: A 120 volt extension cord ran from a power strip inside the office through a hole cut into the window screen and to the elevated platform where it supplied power to the barrel fan.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service:  a) South end of the Warehouse Building #79: The 120 volt cord to a barrel fan had the outer insulation pulled back from the plug-end exposing 1/2 inch of the three individual insulated wires.
