29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    Electrical Hazard Pool Room #1:  On or about February 21, 2019 and at times prior thereto, employees were exposed to an electrical hazard from an exposed breaker box and light switch with missing faceplate located within pool room #1.    Electrical Hazard Pool Room#2:  On or about February 21, 2019 and at times prior thereto, employees were exposed to an electrical hazard from an electrical outlet with a missing face plate in pool room #2.

29 CFR 1910.303(g)(2)(iii): Entrances to rooms and other guarded locations containing exposed live parts were not marked with conspicuous warning signs forbidding unqualified persons to enter.   Electrical Hazard Pool Room #1:  On or about February 21, 2019 and at times prior thereto, employees were exposed to an electrical hazard from an exposed breaker box with a missing cover plate located within pool room #1 that did not have marked signage warning unauthorized persons from entering the pool room.  Electrical Hazard Pool Room #2: On or about February 21, 2019 and at times prior thereto, employees were exposed to an electrical hazard from an outlet that was missing a faceplate.  The pool room did not have marked signage on the exterior doors warning unauthorized persons from entering room.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:   On or about February 21 2019, and at times prior thereto, employees were exposed to respiratory hazards when provided respirator masks upon request, while performing pool cleaning operations without provision of applicable information (Appendix D).
