29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:       KITCHEN CLOSET AREA: On or about February 26, 2019 the employer did not ensure that electrical circuit breakers in panels A, B & C were adequately labeled to identify what they controlled . In the event of an emergency, this condition exposed employees to hazards and injuries associated with not being able to shut-off the electrical power to a piece of machinery or equipment.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    KITCHEN AREA: On or about February 26, 2019 the employer did not ensure that an electrical oven toaster was inspected and removed if there was a defect or evidence of damage that might expose an employee to injury.  The toaster missing a ground prong exposed employees to hazards associated with electrical shock.
