29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):    a)  On or about 03/01/19, at the former Holley High School, located on South Main Street, in Holley, New York, the employer did not utilize an enclosed chute during the disposal of scrap wood and other building materials.  Materials were being tossed out of a second story window 21 feet above ground level into a scrap bin.  Employees were exposed to overhead hazards.       ABATEMENT CERTIFICATION REQUIRED

On or about 03/01/19, at the former Holley High School, located on South Main Street, in Holley, New York, the employer did not utilize fall protection when employees were exposed to falls from an open window, located 21 feet above lower levels.
