29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage:    JOB SITE: 2182 Meadow Forest Drive, Wildwood, MO 63091, on or about 3/5/2019, the employer did not ensure that each cord set were visually inspected for external defects such as deformed or insulation damage or internal damage.   A yellow extension cord was used to power an electric saw, was found lying in the snow and wet ground with cord plug damaged. The workers were exposed to an electrical hazard.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  JOB SITE: 2182 Meadow Forest Drive, Wildwood, MO 63091, on or about 3/5/2019, workers were observed using a yellow extension cord to power an electric saw where the flexible cord was not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  The extension cord was lying on top of the snow and wet ground with the cord's insulation pulled away for the plug with exposed wires.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    JOB SITE: 2182 Meadow Forest Drive, Wildwood, MO 63069, on or about 3/5/2019, workers were observed on the of a 12:12 pitch roof, approximately 12 feet, 2 inches eave to ground level, actively installing sheathing material on the roof, and   were not protected by guardrail systems, safety net system, or personal fall arrest system.  The workers were exposed to a fall hazard.
