29 CFR  1926.501(b)(14):"Wall openings." Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  At 1234 Dougherty Ferry RD, Kirkwood, MO 63122 - On or about October 20, 2021 Employees of Altus Contracting LLC, were exposed to elevated fall hazards over six feet while  working on, at or near wall openings where the outside bottom edge was six feet or more above lower levels and were not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system.

Each employee who was engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  At 1234 Dougherty Ferry RD, Kirkwood, MO 63122 - On or about October 20, 2021 Employees of Altus Contracting LLC, were exposed to elevated fall hazards over six feet while  working on the roof of a residential structure and were not protected by a guardrail system, safety net system or personal fall arrest system.  Altus Contracting LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1382632, citation number 1, item number 2, and was affirmed as a final order on May 13, 2019, with respect to a workplace located at 2182 Meadow Forest Drive, Wildwood, MO 63069.
