29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a) Matt Shisler (dba S & W Enterprises) at the worksite of 2311 North Main Street in Findlay Ohio; The employer did not have a plan or program to identify job site hazards which assist employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during jobsite operations and decreasing the probability of employee injury and/or illness. Employees were exposed to falls off approximately 8 feet to the ground below during re-roofing operations.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Matt Shisler (dba S & W Enterprises) at the worksite of 2311 North Main Street in Findlay Ohio; The employer did not ensure employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternate measures while performing work tasks from elevated working surfaces. Employees were exposed to falls off approximately 8 feet to the ground below during re-roofing operations.

Employees working in this condition were exposed to falls off approximately 8 feet to the ground below.
