Employees were using a scaffold plank located on the second level of a mobile fabricated frame scaffolding system to gain access to a roof, which was located approximately 30 inches in height from the scaffold plank. Employees were using the scaffolding to access the roof for steel stud installation without the use of fall protection system. The condition exposed employees to a fall hazard of approximately 10-foot 6 inches.

The ladder rungs on the end-frames were measured at approximately 22.25 inches in vertical distance from the top of each rung. Employees were climbing the integrated ladder rungs on the end-frames to gain access to the roof. The condition exposed employees to a fall hazard while climbing the scaffolding to gain access to the roof.
