29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: Kousma Insulaton, LLC at the worksite located at 904 Fairwood Blvd., Elyria, Ohio 44035: On or about March 20, 2019, employees were exposed to fall hazards of approximately 10-15 feet in that a 28-ft Werner extension ladder (model D6228-2) was not used as designed. The ladder's fly section was used a single ladder without anti-slip feet to access the siding of a residential structure.
