Employees were exposed to struck by hazards in that the rigging attachment, wrecking ball, cable wire, and a G178 Mixer tire, attached to the Hitachi excavator was not inspected for safe load limits.

No.:AMD300043) in a manner in which the dead end of the wire rope was in contact with the U-Bolt Saddle.
