29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).    ExperCon LLC. on or about June 27, 2019 at the address of 3145 Secor Road, Toledo, Ohio: The employer failed to ensure employees were adequately protected from cave-ins. The trench in this condition exposed employees to cave in hazards when working in a 5 1/2 foot deep excavation, tying in existing underground catch basin pipes without the benefit of a protective system.
