29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Alvarados Construction at 104 Mohawk Cir. Superior, CO 80027: On or before 4/2/19 the employer did not protect employees from injury in that employees working on a two-story single-family home were working without wearing fall protection. Employees were installing new shingles on the roof. This condition exposed employees to a fall hazard of 18'.  Alvarados Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1250876, citation number 1 item number 1a, and was affirmed as a final order on 10/31/2017, with respect to a workplace located at 4256 Clover Lane Brighton, CO 80602.

29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  (a) Alvarados Construction at 104 Mohawk Cir. Superior, CO 80027: On or before 4/2/19 the employer did not protect employees from injury in that employees operating pneumatic nail guns were not wearing safety glasses. This condition exposed employees to eye injury hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a) Alvarados Construction at 104 Mohawk Cir. Superior, CO 80027: On or before 4/2/19 the employer did not protect employees from injury in that the portable extension ladder used to access the roof did not extend at least three feet past the eave of the roof. This condition exposed employees to fall hazards.
