29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    a) On 3/18/2019 an employee was in the process of laminating five sheets of cardboard utilizing an automatic laminator (P2), during the process the bottom strand broke. The employee was trying to feed the broken strand through rollers with the left hand.  The rollers had glue on them, this resulted in the employees left hand being pulled into the rollers.       b) On Laminator P1 employees were exposed to ingoing nip points and rotating parts when accessing the rollers to make adjustments and refeed broken strains of cardboard through a set of doors, while using one hand to manually job the rollers and the other to refeed the tail of the cardboard strain.     c) On Laminator P3 employees were exposed to ingoing nip points and rotating parts when accessing the rollers to make adjustments and refeed broken strains of cardboard through a set of doors, while using one hand to manually job the rollers and the other to refeed the tail of the cardboard strain.     Loroco Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number  1040830, citation number 1, item number1 and was affirmed as a final order on August 26, 2015, with respect to a workplace located at 5000 Creek Road, Blue Ash, OH 45242.    Loroco Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number  1310431, citation number 1, item number1 and was affirmed as a final order on June 16, 2018, with respect to a workplace located at 5000 Creek Road, Blue Ash, OH 45242.
