29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  On or about April 5th, 2019, at the PBT Aquisitions, LLC worksite located at 1200 N 1st St in Fairmont City, IL, Environmnetal Operations, Inc. did not ensure that an employee of Remedy Intelligent Staffing, working for Environmental Operations, Inc., that was voluntarily wearing a tight-fitting, half-mask, air-purifying respirator throughout the site was medically evaluated prior to using the respirator in the workplace.   On or about July 31st, 2019, at the PBT Aquisitions, LLC worksite located at 1200 N 1st St in Fairmont City, IL, the Environmental Operations, Inc. did not ensure that an employee of Remedy Intelligent Staffing, working for Environmental Operations, Inc., that was required to wear a tight-fitting, half-mask, air-purifying respirator at the dryer area was medically evaluated prior to using the respirator in the workplace.
