On or about July 31st, 2019, at the PBT Aquisitions, LLC worksite located at 1200 N 1st St in Fairmont City, IL, the Environmental Operations, Inc. did not ensure that an employee of Remedy Intelligent Staffing, working for Environmental Operations, Inc., that was required to wear a tight-fitting, half-mask, air-purifying respirator at the dryer area was medically evaluated prior to using the respirator in the workplace.
