29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. This violation was observed on or about April 9, 2019 at 300 N. Vista, Edmond, Oklahoma, the employer did not ensure each employee was properly wearing a safety harness with lanyard attached to the proper anchor points while in the JLG Telescopic Boom aerial lift, exposing employees to a fall from approximately 25 ft. to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. This violation occurred on or about April 9, 2019 at that at 300 N. Vista, Edmond, Oklahoma, the employer did not ensure each employee working in aerial lifts and working at heights greater than 10 ft. were provided training in fall hazards and how to properly use fall protection, exposing the employees to fall hazards.
