29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:      A.)  Hodge Foundry, Inc.; molding department - On or about 4/11/19, and at times prior thereto, the company did not provide fall protection for employees when they were performing work activities on top of the flasks.   The condition exposed employees to a fall hazard of up to 4-14 feet.      B.)  Hodge Foundry, Inc.: reclaim area/shaker belt area - On or about 4/11/19, and at times prior thereto, an opening existed in guardrail system by the stairs.  The condition exposed employees to a fall hazard of 5 ft. into the shaker belt pit.

Hodge Foundry, Inc. - On or about 4/11/19, and at times prior thereto, a hole was drilled into the forks of the Clark #2 forklift.  The hole was drilled to allow the forklift to be able to attach and tow a buggy.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place and legible:   A.)  Hodge Foundry, Inc. -  On or about 4/11/19, and at times prior thereto, the Clark #1 forklift had an illegible forklift plate.   The nameplate was unreadable due to it being scratched and faded.   B.) Hodge Foundry, Inc. - On or about 4/11/19, and at times prior thereto, the Clark #2 forklift did not have a forklift plate attached to it.  The forklift had a clean area on it where the plate used to be attached.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:      A.)  Hodge Foundry, Inc.,  core room - On or about 4/11/19, and at times prior thereto, employees were utilizing a bench grinder without a tongue guard.  Tongue guards were missing from both grinding wheels.

Hodge Foundry, Inc. lunchroom area  - On or about 4/11/19, and at times prior thereto, the area in front of and directly below a 480 volt disconnect was used for storage.  Items stored in the area included, buckets and a wood board.
