29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a.) Hodge Foundry, Inc., Mold floor pit area "Dunn Pit area", on or about August 17th, 2022 and times prior thereto:  A section of the Mold floor pit area, directly next to the mold floor walkway, was not guarded against fall hazards.  The guardrail system utilized by the employer had an opening of approximately 19 inches which exposed employees to a potential fall hazard.  The condition exposed employees to a fall hazard into the pit of approximately 4 feet 6 inches.  b.) Hodge Foundry, Inc., Mold floor pit area "Dunn Pit area", on or about August 17th, 2022 and times prior thereto:  The "back" section of the Mold floor pit area, directly next to the north mold floor wall, was not guarded against fall hazards.  The backside of the "Dunn Pit area" did not contain a guard rail system or similar to protect employees from a fall hazard into the pit of approximately 4 feet 6 inches.

Hodge Foundry, Inc., Mold floor pit area "Dunn Pit area", on or about August 17th, 2022 and times prior thereto:  The employer was not inspecting the walking working surface as often as necessary in that, the guardrail system utilized by the employer had an opening of approximately 19 inches.  The opening in the guardrail system exposed employees to a potential fall hazard in the "Dunn Pit area".  The condition exposed employees to a fall hazard into the pit of approximately 4 feet 6 inches.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.134(i)(1)(ii): Breathing air did not meet requirements for Grade D breathing air as described in the ANSI/Compressed Gas Association Commodity Specification G-7.1-1989:  a.) Hodge Foundry, Inc., Abrasive blasting area, on or about August 17th, 2022 and times prior thereto:  Company employees were utilizing supplied breathing air from oil-lubricated air compressors through a RPB Brand Nova 2000 supplied air respirator associated with a Radex airline filter system.  The company did not test the air to ensure that the compressed breathing air, supplied to company employees, met at least the requirements for Grade D breathing air.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
