29 CFR 1926.1053(b)(21): The employer did not ensure that employees used at least one hand to grasp the ladder when progressing up and/or down the ladder:      a) Job site, 940 Ohio River Boulevard, Avalon, PA: On or about April 18, 2019, during construction activities, the employer did not ensure that an employee used ladders safely in that he climbed up an extension ladder while holding a large HVAC vent which prohibited him from keeping at least one hand on the ladder at all times.

29 CFR 1926.1053(b)(22): The employer did not ensure that employee did not not carry any object or load that could cause them to lose balance and fall:   a) Job site, 940 Ohio River Boulevard, Avalon, PA: On or about April 18, 2019, during construction activities, the employer did not ensure that an employee used a ladder safely in that he climbed up an extension ladder while holding a large HVAC vent that could have led to him losing his balance and falling.

29 CFR 1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:   a) Job site, 940 Ohio River Boulevard, Avalon, PA: On or about April 18, 2019, during construction activities, the employer did not ensure that an appropriate system was implemented to identify and correct work place safety hazards. Deficiencies noted during the inspection included but were not limited to, standing on top of a parapet wall with no fall protection, carrying large parts up an extension ladder and stepping sideways onto/off of a step ladder.
